We're looking for a detail-oriented, experienced RN to join our Home Health team in a dedicated Start of Care role, offering top-quartile pay and the tools and support to do your best work every day.


Compensation
  • $39.00 – $46.00 per hour, depending on experience
  • $2,500 Sign On Bonus***
  • Full-time / 40 hours per week

Service Area & Schedule

Cleveland / Northeast Ohio | Monday - Friday, standard business hours


What You'll Do
  • Conduct comprehensive Start of Care (SOC) and Resumption of Care (ROC) assessments for new and returning home health patients
  • Complete accurate and thorough OASIS assessments in compliance with Medicare, Medicaid, and agency standards
  • Develop individualized, evidence-based care plans in collaboration with the referring physician and interdisciplinary team
  • Establish rapport with patients and families from day one, setting clear expectations for the home health episode of care
  • Identify patient goals, barriers to care, and safety concerns during the initial visit
  • Coordinate with RN Case Managers and clinical supervisors to ensure seamless handoff and continuity of care following SOC
  • Maintain accurate and timely documentation in the EMR within required timeframes
  • Participate in team meetings, case conferences, and ongoing professional development

Requirements

Must-Have:

  • Active RN license (Ohio) + current BLS certification
  • Associate or Bachelor's degree in Nursing (BSN preferred)
  • Minimum 2 years of clinical nursing experience, including at least 1 year in a home health setting
  • Demonstrated OASIS experience and strong assessment skills
  • Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
  • Familiarity with Medicare and Medicaid regulations

Preferred / Nice-to-Have:

  • OASIS-D certification or equivalent
  • Familiarity with HCHB or similar home health EMR
  • Experience in a dedicated SOC or admissions nursing role
  • Strong time management skills and ability to manage a high-volume assessment caseload
