Our Home Health team serving Smithfield, Virginia and the surrounding communities is seeking a compassionate and dedicated Registered Nurse to provide high-quality, patient-centered care in the comfort of patients' homes. In this role, you will conduct comprehensive assessments, develop and implement individualized care plans, educate patients and caregivers, and coordinate services with an interdisciplinary team to promote recovery, independence, and overall well-being. What you will do

  • Assesses, plans, implements, and evaluates the total nursing care of assigned patients on caseload on an intermittent visiting basis. Applies standards of practice as it relates to the specialty or patient population served. ( i.e. med surg, hospice, IV therapy). Develops the initial and ongoing comprehensive assessment of the clients needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points.
  • Supervises and delegates non-professional staff to ensure optimum patient care. Recognizes and responds to emergency situations. Evaluates own needs for support and uses identified systems and resources to meet patient care needs. Supervises LPN/LVN's, and Home Health Aides in providing services.
  • Completes all required clinical documentation consistently per RHS standards which includes completing and "locking" patient notes and verbal orders, as well as completing certifications and recertification's after the department's multi-disciplinary team meetings. Completes required documentation no later than the last day of the month in order to provide timely billing.
  • Implements/initiates a plan of care and revises when necessary, provides services and/or treatments requiring specialized nursing skill, councils the client and family in meeting nursing need, coordinates services, informs the physician and other staff of changes in the client's needs, and plans for the patient to be discharged from our services. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient.
  • Participates in case conferences. Provides education to client and family. Participates in training and support of all team members, volunteers and students as needed.

Licenses and Certifications

  • Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
  • Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job
  • Valid Drivers License Required
