Position Overview Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in WellSky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes. Safe Life utilizes Andy AI , which assists our clinicians with OASIS completion and submission within 24 hours.

Responsibilities

  • Provide strong clinical skills, excellent communication abilities, and a passion for patient-centered care

  • Understands and adheres to established policies and procedures

  • Conduct comprehensive assessments of patients' health status and needs in their home environment

  • Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition

  • Develop and implement individualized care plans based on patient assessments and medical history

  • Administer medications and treatments as prescribed, ensuring adherence to safety protocols

  • Monitor patients’ progress, documenting changes in condition and response to treatment

  • Provide education to patients and families/significant others regarding disease process and management, medication regimen, pain management, fall prevention, infection control, proper diet / exercise, when to contact pharmacy / physician / SN and when to seek emergency care.

  • Collaborate with interdisciplinary teams to coordinate care, including discharge planning and referrals to other services as needed

  • Maintain accurate medical records and documentation in compliance with legal and regulatory standards

  • Respond promptly to patient inquiries or concerns, providing support and guidance as necessary

  • Attends staff meetings, patient care conferences and in-services as scheduled
