Are you a compassionate, experienced RN who loves making personal connections and helping patients understand their options at a vulnerable time? As a Hospice Admissions Nurse, youโll be the face of our careโhelping families say โyesโ to compassionate end-of-life support when it matters most.
What You'll Do as a Hospice RN Admissions Nurse:
โข Report directly to the Admissions Manager and serve as a key ambassador for our hospice services
โข Visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
โข Explain hospice philosophy and services clearly, compassionately, and with clinical confidence
โข Guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
โข Collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
โข Use the nursing process to assess patient needs, support care planning, and document clinical findings
โข Build trust and provide emotional reassurance to patients and families as they make this important decision
โข Accurately complete required documentation and ensure compliance with state and federal regulations
โข Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff
About You:
Qualifications - What Youโll Bring:
โข Active RN license in the state of employment (or eligible to obtain)
โข 1+ year of general nursing experience
โข A hospice heart: compassionate, empathetic, and deeply committed to patient-centered care
โข Strong assessment, communication, and interpersonal skills
โข Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
โข Valid driverโs license, auto insurance, and reliable transportation
โข Current CPR certification
Preferred Experience (Not Required):
โข 2+ years of hospice, home health, healthcare admissions, or medical sales
โข Bachelorโs Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification
โข Background in case management, care coordination, or healthcare outreach
โข Experience with electronic medical records and compliance documentation
