Are you a compassionate, experienced RN who loves making personal connections and helping patients understand their options at a vulnerable time? As a Hospice Admissions Nurse, youโ€™ll be the face of our careโ€”helping families say โ€œyesโ€ to compassionate end-of-life support when it matters most.

What You'll Do as a Hospice RN Admissions Nurse:


โ€ข Report directly to the Admissions Manager and serve as a key ambassador for our hospice services
โ€ข Visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
โ€ข Explain hospice philosophy and services clearly, compassionately, and with clinical confidence
โ€ข Guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
โ€ข Collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
โ€ข Use the nursing process to assess patient needs, support care planning, and document clinical findings
โ€ข Build trust and provide emotional reassurance to patients and families as they make this important decision
โ€ข Accurately complete required documentation and ensure compliance with state and federal regulations
โ€ข Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff

About You:

Qualifications - What Youโ€™ll Bring:

โ€ข Active RN license in the state of employment (or eligible to obtain)
โ€ข 1+ year of general nursing experience
โ€ข A hospice heart: compassionate, empathetic, and deeply committed to patient-centered care
โ€ข Strong assessment, communication, and interpersonal skills
โ€ข Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
โ€ข Valid driverโ€™s license, auto insurance, and reliable transportation
โ€ข Current CPR certification

Preferred Experience (Not Required):

โ€ข 2+ years of hospice, home health, healthcare admissions, or medical sales
โ€ข Bachelorโ€™s Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification
โ€ข Background in case management, care coordination, or healthcare outreach
