Job description

Registered Nurse (RN) Case Manager

Seeking an experienced and compassionate Registered Nurse (RN) Case Manager to provide skilled home health services throughout Summit, Portage, and Medina Counties. This role is responsible for managing a patient caseload, coordinating care, performing assessments, developing individualized care plans, and ensuring patients receive high-quality care in the comfort of their homes. The ideal candidate will have prior home health experience and be comfortable working independently in a field-based setting.

Schedule & Work Details

Agency: Caretenders Home Health Uniontown

Coverage Area: Summit, Portage, and Medina Counties, Ohio

Schedule: Monday through Friday, 8:00 AM to 4:30 PM

Hours: 8 hours per day, 5 days per week

Pay Rate: $51.00 - $53.00 per hour

Productivity Expectation: 30 points per week

On-Call Requirement: One weeknight rotation per month and one weekend per month

Mileage Reimbursement: Reimbursed between patient visits; mileage from home to the first patient and from the last patient to home is not reimbursed

Key Responsibilities
  • Serve as the primary RN Case Manager for an assigned home health patient caseload.
  • Conduct comprehensive patient assessments and ongoing evaluations.
  • Develop, implement, and update individualized plans of care in collaboration with physicians and other healthcare professionals.
  • Provide skilled nursing care in accordance with physician orders and agency policies.
  • Monitor patient progress and communicate changes in condition to the appropriate members of the care team.
  • Coordinate services with therapists, home health aides, physicians, and other interdisciplinary team members.
  • Educate patients and caregivers regarding medications, disease management, treatment plans, and home safety.
  • Maintain accurate, timely, and compliant documentation.
  • Participate in case conferences and quality improvement initiatives.
  • Ensure compliance with all agency, state, federal, and payer requirements.
  • Participate in the agency's on-call rotation schedule.
Required Qualifications
  • Active Registered Nurse (RN) license in the State of Ohio.
  • Previous home health nursing experience required.
  • Strong clinical assessment and case management skills.
  • Valid driver's license, reliable transportation, and current automobile insurance.
  • Ability to work independently and manage a field-based caseload.
  • Excellent communication, organizational, and time management skills.
Preferred Qualifications
  • Experience with Homecare Homebase (HCHB).
  • Knowledge of OASIS assessments and Medicare home health regulations.
  • Strong care coordination and interdisciplinary collaboration skills.
  • Previous experience managing a home health patient caseload.
  • Commitment to delivering exceptional patient-centered care.
