Registered Nurse (RN) – Home Health

Pay: $55–$60/hour. Full‑Time position. Coverage area: Ventura, Santa Paula, Oxnard, Camarillo.

Job Summary

The Home Health Registered Nurse (RN) plans, organizes, and directs home health services while providing high‑quality, patient‑centered care. The RN collaborates with an interdisciplinary team to develop individualized care plans and deliver skilled nursing services in the patient’s home. This position is ideal for nurses who value autonomy, patient interaction, and the ability to make a direct impact.

Responsibilities
  • Develop, implement, and revise individualized plans of care with patients, caregivers, and providers
  • Provide skilled nursing care, including preventive and rehabilitative services per physician orders
  • Deliver patient and caregiver education to promote independence and safety
  • Offer counseling and support to patients and families
  • Maintain accurate and timely clinical notes and documentationCommunicate regularly with physicians and healthcare team members regarding patient status
  • Identify and coordinate discharge planning needs
  • Ensure care is delivered in compliance with all regulatory and company standards
Qualifications
  • Active California RN license (required)
  • At least 2 years of nursing experience, preferably in home health or public health
