Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Home Healthcare team. In this role, the LPN will provide direct nursing care to patients in their homes under the supervision of a Registered Nurse or physician. The ideal candidate has prior home healthcare experience, strong clinical skills, and a passion for delivering high-quality, patient-centered care in a home setting.
This position requires travel to patient homes throughout Atlanta, Cobb, Gwinnett, Henry, and Clayton Counties.
Responsibilities
  • Provide skilled nursing care to patients in their homes in accordance with the plan of care
  • Assist with patient assessments and report findings to the supervising RN
  • Administer medications and treatments as prescribed
  • Monitor and document patient conditions, progress, and responses to care
  • Observe and report any changes in patient condition to the RN or supervisor
  • Educate patients and families on medications, treatments, and daily care needs
  • Maintain accurate and timely documentation in compliance with agency and state requirements
  • Follow all infection control, safety, and home healthcare protocols
Qualifications
  • Current Licensed Practical Nurse (LPN) license in the state of Georgia
  • Previous home healthcare experience REQUIRED

  • Knowledge of basic nursing practices and home health procedures
  • Excellent interpersonal, communication, and patient care skills
  • Ability and willingness to travel to Atlanta, Cobb, Gwinnett, Henry, and Clayton Counties

  • Valid driver’s license and reliable transportation
  • Current First Aid and CPR certification

  • Strong organizational skills and attention to detail
