Casual Home Hospice Registered Nurse (RN)

The Casual Home Hospice Registered Nurse (RN) in Manitowoc County plays a vital role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses in the comfort of their own homes. This position focuses on managing and coordinating comprehensive hospice care plans that address physical, emotional, and spiritual needs, ensuring quality of life and dignity for patients and their families on an as-needed basis. The RN will collaborate closely with interdisciplinary teams, including physicians, social workers, and chaplains, to provide holistic support and symptom management. Flexibility and responsiveness are key, as the casual nature of the role requires adapting to varying patient needs and schedules. Ultimately, the RN serves as a trusted healthcare professional who advocates for patient comfort and supports families through the end-of-life journey with empathy and clinical expertise.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of Wisconsin.

  • Ability to cross train and float to other areas within SSRCH as needed.

  • Conduct thorough patient assessments to evaluate physical symptoms, psychosocial status, and care needs in the home setting.
  • Develop, implement, and regularly update individualized hospice care plans in collaboration with the interdisciplinary team.
  • Administer medications and treatments as prescribed, monitor patient responses, and adjust care plans accordingly.
  • Provide education and emotional support to patients and their families regarding disease progression, symptom management, and end-of-life care options.
  • Document all patient interactions, care plans, and clinical observations accurately and timely in compliance with regulatory standards.
  • Coordinate with other healthcare providers and community resources to ensure seamless continuity of care.
  • Respond promptly to patient and family needs, including after-hours and emergency situations as required by the casual schedule.
