Roles & Responsibilities
- Assess and respond to the physical, psychosocial, and environmental needs of patients in their homes.
- Implement, document, and evaluate the Plan of Care (POC) to ensure continuity of care.
- Educate patients and families on care protocols, medications, and safety practices.
- Supervise home health aides and provide clinical direction to ensure quality service delivery.
