Job description

Eden HHC is looking for experienced Registered Nurses to join our dedicated team. We are looking for RN's in the Los Angeles, San Bernardino, Riverside County and Orange County. The candidate must have minimum of 1 year professional home health nursing experience. We use Kinnser/Wellsky for EMR, can train if needed.

Position is for Per Diem Field RN. Must be willing to travel to visit patients for admission, re-certification, resumption of care, supervisory visit, discharge visit, IV infusion and other RN required visits to different cities.

Candidates must have:

Must be flexible with daily schedule

Must be comfortable with fast pace work environment

Willing to be on the road, as this is a visiting/field position

Current active RN license in the state of California

Current CPR, Health clearance, TB clearance

Minimum of one year of RN work experience, home health preferred

Must pass criminal background check

Reliable transportation

Driver’s license without restrictions

Proficiency in OASIS charting and timely submission of documents, will train if needed

Proof of COVID-19 vaccinations, unless exempt

Must have team player attitude

Responsibilities:

Complete initial and on-going comprehensive assessments of the patient’s needs

Provide service and /or treatments requiring substantial and specialized nursing skills

Initiate appropriate preventative and rehabilitative nursing procedures and therapy referrals

Educate and support the patient, family and caregiver in meeting nursing and related needs

Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.

Initiates plan of care and participates in case conferences
