Job description

Registered Nurse Home Health PRN Redding

This position is paid on a per-point basis.

Responsibilities:

โ€ข Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial needs.

โ€ข Develop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team (including physicians, therapists, social workers, and aides).

โ€ข Provide direct skilled nursing care, such as wound care, medication management, IV therapy, symptom management (especially pain and comfort in hospice), and patient/caregiver education.

โ€ข Offer education and emotional support to patients and their families/caregivers based on individual needs.

โ€ข Supervise LPNs/LVNs, home health aides, and other support staff.

โ€ข Participate in on-call rotations, admissions, or urgent response as needed (common in hospice roles).

โ€ข Coordinate care activities, including visit scheduling and documentation to ensure compliance with regulations.

โ€ข Monitor patient progress, respond to changes in condition, and adjust care plans accordingly.

โ€ข Maintain accurate clinical records and ensure care aligns with agency policies, professional standards, and regulatory requirements.

Qualification:

  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN.

Required Certifications and Licensures:

  • Licensed to practice as a registered nurse in the state of agency operation.
  • Must possess and maintain valid CPR certification while employed in a clinical role.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
