RN Case Manager Job Responsibilities:

  • Assesses, plans, implements and evaluates the care provided to all patients assigned.
  • Provide client care according to plan of care
  • Reviews, interprets, transcribes and carries out physician orders
  • Prepares and maintains clinical documentation.
  • Provides legible, clear written or electronic documentation of all patient care provided, medications administered, and the patient's response to care.
  • Supervises professional and non-professional personnel providing patient care. Provides supervision to others through task assignments, direction, evaluation and feedback of assigned tasks.
  • Implements infection control and safety precautions.
  • Attend team meetings, training, and assists teammates as needed.

RN Case Manager Job Requirements:

  • Reliable Transportation
  • CPR Card (BLS)
  • TB Test (current)
  • Health Screening
  • Must pass criminal background check, abuse background check and drug screening.
  • Must provide a valid Driver's License, current Auto Insurance, and Motor Vehicle Report
  • Current RN licensed in the state of Illinois.(must be in good standing)
  • Minimum 2 years of recent acute care and Home Health experience.
