Position Overview
Seeking an experienced and autonomous Registered Nurse (RN) to provide skilled home health services throughout Lexington and the surrounding service area. This position is responsible for delivering high-quality nursing care, completing comprehensive assessments and OASIS documentation, coordinating care across the interdisciplinary team, and promoting positive patient outcomes. The ideal candidate will have prior home health experience, strong OASIS knowledge, and proficiency with Homecare Homebase (HCHB).
Schedule & Work Details
- Coverage Areas: Bourbon, Clark, Estill, Fayette, Franklin, Harrison, Jessamine, Nicholas, and Woodford Counties
- High-need coverage areas include Estill, Clark, and Nicholas Counties
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 32+ hours
- Pay Rate: $51.00 - $53.00 per hour
- Productivity Expectation: 30 points per week
- On-Call Requirement:
- Approximately 1 weekend every 4 weeks
- 5-6 weekday on-call shifts per month
- Mileage Reimbursement: Reimbursed between patient visits only; travel from home to the first patient and from the last patient to home is not reimbursed
Required Meetings:
Daily morning Teams call at 8:30 AM
Weekly case conference every Wednesday at 8:30 AM
Key Responsibilities
- Deliver Skilled Nursing Care
- Provide comprehensive skilled nursing assessments, interventions, treatments, and patient education in the home setting.
- Perform complete medication reconciliations during every patient encounter.
- Develop, implement, and revise individualized plans of care based on physician orders and patient needs.
- Monitor patient progress and communicate significant changes in condition to physicians and appropriate care team members.
- Manage a Multi-County Caseload
- Schedule and coordinate patient visits efficiently across a large geographic territory.
- Adapt to changing referral volumes, census demands, and staffing needs.
- Provide coverage throughout multiple service areas as assigned.
- Manage daily travel while maintaining productivity and quality standards.
- Complete OASIS & Clinical Documentation
- Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Follow-Up, and Discharge assessments.
- Complete accurate and timely OASIS documentation within required deadlines.
- Maintain documentation that supports clinical necessity, regulatory compliance, reimbursement requirements, and quality patient care.
- Utilize Homecare Homebase (HCHB) for documentation and patient management activities.
- Ensure Regulatory & Quality Compliance
- Adhere to Medicare Conditions of Participation, agency policies, and state regulations.
- Follow infection prevention, patient safety, and risk management standards.
- Participate in audits, quality improvement initiatives, and performance enhancement activities.
- Ensure all documentation and care delivery meet compliance requirements.
- Coordinate Interdisciplinary Care
- Collaborate with physicians, therapists, social workers, office staff, and other healthcare professionals.
- Communicate patient needs, barriers to care, and plan of care updates promptly.
- Participate in weekly case conferences and other care coordination activities.
- Support seamless communication across the healthcare team to enhance patient outcomes.
- Promote Positive Patient Outcomes
- Identify and address patient risks including falls, medication concerns, and chronic disease management needs.
- Educate patients and caregivers to improve self-management skills and reduce avoidable hospitalizations.
- Support agency quality metrics, patient satisfaction goals, and positive clinical outcomes.
- Maintain Productivity & Flexibility
- Meet productivity expectations while maintaining exceptional patient care and documentation standards.
- Respond to urgent patient and staffing needs throughout the service area.
- Demonstrate flexibility and willingness to accept assignments based on agency needs.
- Contribute to a team-focused environment that prioritizes patient care and organizational success.
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license.
- Minimum of 1-2 years of home health nursing experience.
- Experience with Homecare Homebase (HCHB) is required.
- Strong knowledge of OASIS assessments and documentation requirements.
- Current BLS certification.
- Valid driver's license, reliable transportation, and proof of insurance.
- Strong head-to-toe assessment skills and medication reconciliation experience.
- Ability to work independently and effectively manage a field-based caseload.
