Position Overview:
We are seeking a skilled and compassionate Registered Nurse (RN) to join our home health team. The ideal candidate is self-motivated, organized, and committed to delivering personalized care. You’ll be responsible for assessing patients, developing care plans, coordinating with physicians, and educating patients and families—all while working independently in the field.
Key Responsibilities:

  • Provide skilled nursing care to patients in their homes
  • Perform comprehensive assessments and develop individualized care plans
  • Collaborate with physicians, therapists, and other clinicians to ensure coordinated care
  • Educate patients and caregivers on disease management and medication administration
  • Document all care accurately and in a timely manner

Qualifications:

  • Active RN license in CA
  • Minimum of 3 year of nursing experience (home health preferred, but not required)
  • Current CPR certification
  • Valid driver’s license and reliable transportation
