Job description


This position is for a Registered Nurse who wants to continue using their clinical knowledge without providing direct or physical patient care.
The RN in this role completes in-home visits by talking with clients and families, review health history, and develop clear, practical care plans for non-medical in-home caregivers to follow. The work is focused on assessment, communication, and care planningnot clinical tasks.

What you will be doing
  • Travel to clients homes to complete scheduled, non-clinical nursing assessments
  • Conduct verbal and observational assessments with clients and family members
  • Review medical history, diagnoses, medications, functional abilities, and daily routines
  • Identify risks, support needs, and appropriate non-medical services
  • Develop and update individualized care plans for in-home caregivers to follow
  • Serve as a clinical resource to the care team regarding the clients plan of care
  • Maintain accurate and timely documentation of assessments and care plans
