Physical Therapist Opportunity

Pride Health is hiring a Physical Therapist to support our client's patients in Mountlake Terrace, WA, with coverage in King County and Snohomish County (Seattle to Mill Creek). This is a 13-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Job Details:

Job Name: Home Health Physical Therapist

Contract type: Contract

Location: Mountlake Terrace, WA

Coverage Area: King County and Snohomish County (Seattle to Mill Creek)

Duration: 13-week Contract (Possibilities of extension)

Schedule: 8:00am - 4:30pm, Monday through Friday (5 days/wk.)

On Call/ weekend Requirement: No

Mileage: Reimbursement provided (from first patient to last patient)

Job Responsibilities:

  1. Perform comprehensive OASIS assessments using HCHB (Homecare Homebase), including Start of Care (SOC), recertification, 30-day reassessments, and discharge evaluations in accordance with physician orders and agency protocols.
  2. Conduct skilled physical therapy evaluations assessing strength, mobility, balance, pain, gait, and functional limitations to develop individualized Plans of Care (POC).
  3. Provide skilled physical therapy interventions during routine and follow-up (revisit) appointments to maximize patient independence and functional outcomes.
  4. Perform re-evaluations to assess patient progress, modify treatment plans, and update functional goals as clinically appropriate.
  5. Supervise and collaborate with Physical Therapist Assistants (PTAs), ensuring delegated treatments are completed appropriately and in compliance with regulatory standards.
  6. Educate patients and caregivers on home exercise programs, fall prevention, safety techniques, mobility training, and disease management strategies.
  7. Coordinate care with physicians, interdisciplinary team members, caregivers, and support staff to ensure continuity of care and optimal patient outcomes.
  8. Complete timely, accurate, and compliant clinical documentation within HCHB in accordance with Medicare, OASIS, and organizational standards.
  9. Conduct discharge visits including final assessments, patient/caregiver education, and transition planning to support safe continuation of care.

Qualifications:

  • Graduate of an accredited Physical Therapy program (master's or higher).
  • Active, unrestricted Physical Therapist (PT) license.
  • Minimum 1 year of clinical experience (home health experience/ HCHB required).
  • Valid driver's license and reliable transportation.
