Job description

Parx Home Health Care is looking for a Physical Therapist to join our team.

The home health therapist will be responsible for providing high quality and coordinated care to patients in their homes and communities. The ideal candidate will have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives.

What you’ll be doing:

  • Conduct comprehensive physical therapy evaluations and create personalized treatment plans to help patients achieve their goals.
  • Teach patients and their families how to properly perform exercises and activities to promote recovery and improve mobility.
  • Document patient progress, including changes in condition, treatment plan, and goals, and communicate updates to other healthcare professionals.
  • Coordinate care with other medical professionals, including physicians, nurses, social workers, and speech therapists, to ensure the most effective treatment is being delivered.
  • Adhere to established policies and procedures, including infection control and safety protocols.
  • Educate patients and their families on injury prevention and self-care techniques to reduce the chances of relapse or recurring injuries.
  • Maintain accurate and thorough records of patient interactions and treatment plans in accordance with state and national regulations.

Requirements

What we expect from you:

  • Graduate from an accredited physical therapy program with a valid Physical Therapy license in the state of Florida.
  • Minimum of one year of experience in physical therapy, preferably in a home health care setting.
  • Demonstrated ability to create effective physical therapy treatment plans that promote patient recovery.
  • Effective communication skills, both verbally and in writing, to effectively communicate with patients, families, and other healthcare professionals.
  • Ability to work independently and collaboratively as part of a larger healthcare team.
  • Ability to read and interpret physician orders and implement treatment according to orders.
  • A passion for helping patients achieve their best possible outcomes, through compassionate care and evidence-based treatment.
