Job description

Job Title: Registered Nurse (RN) – Home Health

Location: South Bend
Job Type: Full-Time/Part-Time/PRN
Reports To: Clinical Supervisor / Director of Nursing

Position Summary:

We are seeking a dedicated and knowledgeable Registered Nurse (RN) to provide skilled home health services. In this role, you will deliver compassionate, patient-centered care in the home setting while ensuring compliance with Indiana Medicaid requirements. You will work closely with patients and care teams to implement individualized care plans that promote health, independence, and an enhanced quality of life.

Key Responsibilities:
  • Deliver skilled nursing services, including wound care, medication administration, chronic disease management, and health education.

  • Ensure all services are medically necessary and documented to meet Medicaid reimbursement standards.

  • Coordinate care with physicians, case managers, and other interdisciplinary team members.

  • Educate patients and caregivers on disease processes, medication regimens, and self-care strategies.

  • Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).

  • Participate in Medicaid audits, quality assurance activities, and case conferences.

  • Advocate for patient needs and assist with accessing community resources and support services.

Qualifications:
  • Current and unrestricted Registered Nurse (RN) license in the state of Indiana.

  • Minimum of 1 year of nursing experience; home health and Medicaid experience strongly preferred.

  • Knowledge of Indiana Medicaid policies, documentation standards, and prior authorization processes.

  • Current CPR certification.

  • Valid driver’s license and reliable transportation.

  • Strong clinical judgment, communication, and organizational skills.

  • Experience with electronic documentation systems preferred.

Working Conditions:
