As a Physical Therapist at Periscope Physical Therapy, you will:

• Direct and implement personalized physical therapy treatment plans aimed at preventing disability and relieving pain.
• Supervise and collaborate with Physical Therapy Assistants to ensure effective execution of care plans.
• Conduct thorough evaluations of clients’ functional abilities through diagnostic assessments, creating tailored rehabilitation strategies.
• Assess the home environment and recommend necessary modifications to enhance independence and safety for clients.
• Deliver prescribed physical therapy services, including manual therapeutic exercises and the application of various treatment modalities.
• Participate in the interdisciplinary team to develop, review, and refine care plans, ensuring goals are met efficiently.
• Document and communicate progress and changes in clients' conditions effectively with team members and physicians.
• Provide education to clients and families about their rehabilitation programs and physical therapy equipment usage.
• Maintain accurate clinical records, ensuring compliance with OASIS and other regulatory requirements.

Our patient population primarily consists of individuals requiring home-based rehabilitation services, including the elderly, those recovering from surgeries, and patients with chronic conditions. Applicants should possess the following qualifications:

• Doctorate in Physical Therapy (DPT) with valid state licensure in good standing.
• Minimum of two years of experience in a clinical setting, preferably within home care or acute care environments.
• Current CPR certification, valid driver’s license, and automobile insurance.
• Strong interpersonal skills and a passion for enhancing patient outcomes through quality care.
