TITLE: RN Charge Nurse
REPORTS TO: Director of Nursing
Job Summary: To ensure that the highest level of resident functioning and care is obtained through development of resident specific care plans. To coordinate care conferences with other departments, residents and/or families. To coordinate Medicare activities pertaining to residents and communicate Medicare needs to other departments. To assist with resident care as needed to ensure resident's needs are being met.
Qualifications: Must have a current/active LPN or RN license with the State of Minnesota. Must keep up with CEU's for license renewal. Must know and understand Universal Precautions. The charge nurse is also required to have an ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.
General Responsibilities:
  • Must be able to transfer, turn, or assist resident to or from beds, wheelchairs, mechanical lifts, toilets, tubs, showers, and occasionally the floor (for resident weights up to 250 and infrequently above).
  • Perform tasks using resident care equipment, such as pins, buttons, snaps etc.
  • Continual walking and standing are required. As well as reaching and bending.
  • Perform tasks to administer medications (oral, injection, IV, topical, transdermal)
  • Participates in care conferences and communicates resident needs to other appropriate personnel.
  • Assist with quarterly and as needed assessments (fall, pain, skin, bowel and bladder, safety, smoking, TD).
  • Reports any changes in resident condition to family and physician.
  • Works with other nursing staff, providing direct care when needed.
  • Notifies administrator, DON, and social worker of vulnerable adult issues.
  • Transcribes orders accurately and completely.
  • Documents PRN's properly.
  • Follows correct med pass procedure and signs off all medications and treatments.
  • Follows controlled substance policy and procedures.
  • Promotes a positive image of long-term care.
  • Serves as a good role model for nursing staff.
  • Receptive and helpful to nursing staff requesting information and assistance.
  • Maintains good working relationships with staff, residents, and their families.
  • Protects privacy of residents and employees.
  • Brings serious problems to the attention of the department supervisor or the administrator.
  • Distinguish sound via stethoscopes and respiratory status of residents.
  • Touch-Handling and feeling is required continuously for palpation, operating and maintaining equipment and providing medical treatments for abnormal conditions such as wound care resident care and medication administration.
  • Concentration-be able to concentrate on moderate and/or fine detail with frequent interruptions and attend to tasks lasting from 10-60 minutes.
  • Able to remember multiple tasks, messages and information.
  • Conceptualization-must be able to understand and relate to multidisciplinary theories behind several related concepts.
  • Other duties as assigned.
