About the Role
One Home Health is seeking a part-time Licensed Practical Nurse (PNP) nurse to join our growing Indiana team! Openings are available in Lake County (Crown Point, Hobart, Gary, Hammond, Dyer, etc.), Porter County (Valparaiso, Portage, Porter, Hebron, etc.), and the surrounding areas.
What You'll Do
  • Provide professional nursing care in accordance with the Nurse Practice Act under the supervision of a Registered Nurse; promptly report changes in patient condition per agency guidelines
  • Promote and maintain a safe environment for clients, families, and coworkers
  • Perform all duties in compliance with agency policies, procedures, and professional standards
  • Assist in coordinating home care services, attend required meetings, and follow established reporting protocols
  • Operate glucometers and INR monitoring devices with basic proficiency
  • Deliver basic wound care in accordance with established care plans
  • Complete additional duties as assigned

What You Bring
  • Graduate of an accredited professional nursing program
  • Current Licensed Practical Nurse (LPN) license in applicable state(s)
  • At least 1 year of acute care experience or equivalent (community/home health preferred)
  • Current CPR certification
  • Strong written and verbal communication skills with effective interpersonal abilities
  • Experience using electronic medical records (EMR) systems
  • Proficient with tablet-based documentation and email/text communication
  • Ability to interact tactfully with clients and community members
  • Strong organizational and time management skills
  • Valid driver's license, reliable insured vehicle meeting state/agency requirements, and a clean driving record
