This position allows for a flexible work setting as the counselor can operate from home while also engaging with patients in a structured outpatient office environment. In this role, you will be expected to:

• Partner with primary care providers to recognize, diagnose, and manage mental health and psychosocial issues.
• Conduct thorough patient intakes that include diagnostic and functional evaluations to inform subsequent treatment recommendations.
• Deliver brief yet impactful interventions to patients, aiding their mental health recovery process.
• Advise healthcare team members on cases better suited for specialty mental health facilities and ensure appropriate referrals are made.
• Initiate follow-up communications with patients to monitor their progress, adjusting treatment strategies as necessary.
• Develop tailored relapse prevention plans that help patients maintain stability in their mental health.
• Identify patients at risk and collaborate in crafting preventative plans to avert deterioration of their mental condition.
• Document patient progress, treatment plans, and relevant diagnostic data in their health records, ensuring accurate and confidential information flow.
• Maintain open and informative communication with primary care providers about patient needs and achievements.
• Implement behavioral change strategies for patients at risk for physical disorders, promoting healthier lifestyle choices.
• Empower patients by providing them with essential self-management skills and educational resources.
• Consult with management and colleagues regarding behavioral health services and contribute to program evaluations and outcomes.
• Assist patients in adhering to medical treatments initiated by their primary care providers, which may include strategies to mitigate medication side effects.
• Uphold the Code of Ethical Conduct for Clinical Professionals while carrying out assigned tasks.

You will work with a wide range of patients, including those facing complex mental health challenges and psychosocial difficulties. Your efforts will focus on delivering effective counseling and therapeutic interventions tailored to individuals and families from diverse backgrounds. By understanding the unique social determinants of health affecting each patient, you will help bridge gaps in care and foster improved health outcomes.

The ideal candidate will possess the following qualifications:

• A Master’s Degree in Clinical Counseling from an accredited program.
• A current Licensed Professional Counselor (LPC) license to practice in Georgia.
• Proficient understanding of mental health assessment, treatment planning, developmental psychopathology, and family therapy methods.
• Strong knowledge of DSM-IV diagnoses and behavioral therapy techniques.
• Preferred bilingual proficiency in English and Spanish for enhanced patient interactions.
• Robust mental and physical health to meet the role’s demands.
• In-depth awareness of community resources and their effective utilization in supporting patient care.
• Exceptional ability to evaluate patient needs and adjust services accordingly.
• Strong skills in clinical record maintenance and reporting.
• Ability to establish collegial relationships within an interdisciplinary team.
• Excellent communication skills, enabling you to express concepts clearly and effectively.
• A high level of judgment and autonomy in decision-making processes.

Experience in a primary care setting, coupled with a proven track record in managing diverse mental health cases, will be considered a significant advantage. Familiarity with utilizing electronic health record systems to document patient interactions and treatment plans effectively is also preferred.

This position offers an attractive salary range of USD 87,000 – 105,000 per year, commensurate with experience and qualifications.
