Job Description:
The RN in Home Health will primarily manage and oversee care for approximately 30 adult patients, conducting start of care, recertifications, and daily visits using Homecare Homebase (HCHB) charting systems. This role requires licensure in the state of Kentucky and a minimum of one year of experience on a Medsurg floor within a hospital setting. Home Health experience and familiarity with managing patients in home settings without clinical involvement are essential.
Responsibilities:
  • Case manage roughly 30 adult patients utilizing the Homecare Homebase (HCHB) system.
  • Conduct start of care, recertifications, and daily visits achieving an average of 6.5 points a day.
  • Collaborate with clinical managers or educators for field support available 24/7.
  • Work five 8-hour shifts with additional weekend rotations depending on staffing needs.
Requirements:
  • Must have 1 year of experience on a Medsurg floor in a hospital setting.
  • Valid RN license in the state of Kentucky upon submission.
  • BLS certification required (AHA).
  • Experience with Homecare Homebase (HCHB) charting system required.
