Are you a motivated and dynamic registered nurse looking for an immediate opportunity in the home health industry? Do you enjoy providing excellent patient care and making a positive impact in the lives of individuals and families? If your answer is yes, then we have the perfect job for you!

Overview:

The home health registered nurse plans, organizes, and directs/coordinates home care services. This role requires experience in nursing, with an emphasis on community health education/experience. As a professional nurse, you will utilize the resources of the community to plan and direct services that meet the needs of individuals and families within their homes and communities.

Duties & Responsibilities:

  • Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)
  • Provide preventative and rehabilitative nursing procedures as ordered by the physician
  • Offer patient, caregiver, and family counseling and education
  • Prepare clinical notes and communicate with the physician and other healthcare practitioners
  • Identify discharge planning needs and implement prior to patient discharge
  • Communicate with the primary physician and community health related persons to coordinate the care plan
  • Participate in quality assessment and performance improvement programs
  • Supervise home health aides and evaluate their care

Job Requirements:

  • Graduated from an approved school of nursing and licensed in the state of practice
  • Minimum of two years of experience, with preference given to public health or home care nursing
  • Current CPR certification
  • Valid driver's license and insured automobile in good working order
  • Self-directed and able to work with minimal supervision
  • Excellent observation, problem-solving, and communication skills
  • Ability to organize and prioritize workload independently
  • Prolonged walking or standing, lifting, and physical agility
