Job Title:* Physical/Occupational Therapist, PRN
Job Summary:
We are seeking a compassionate and experienced Physical Therapist to join our Home Health Care team. As a Home Health Care Physical Therapist, you will provide high-quality, patient-centered care to patients in their homes, focusing on improving mobility, strength, and function.
Responsibilities:
Conduct initial and ongoing assessments of patients' physical therapy needs
Develop and implement individualized treatment plans
Provide skilled physical therapy interventions, including exercises, manual therapy, and modalities
Educate patients and caregivers on proper techniques and strategies
Monitor patient progress and adjust treatment plans as needed
Collaborate with interdisciplinary teams, including physicians, nurses, and occupational therapists
Document patient information accurately and timely
Participate in case conferences and quality improvement initiatives
Requirements:
Bachelor's or Master's degree in Physical Therapy
Current PT licensure in the state of Florida
Minimum 1-2 years of experience in physical therapy, preferably in home health care
Strong clinical skills, critical thinking, and problem-solving abilities
Excellent communication, interpersonal, and patient education skills
Ability to work independently and as part of a team
Reliable transportation and ability to travel to patient homes
Preferred Qualifications:
Home health care experience or certification (e.g., HHC PT, OASIS)
Knowledge of Medicare regulations and guidelines
Experience with electronic medical records (EMRs, Wellsky/Kinnser) and mobile documentation systems
