This is a home health RN position that travels between the homes of our patients residing in the Greater Seattle area in King County.

A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager via physician orders. This position supervises and coordinates the care provided by LPN's and CNA's assigned to the RN-CM's patients. Provides feedback to Director of Clinical Services regarding performance of supervised LPN's and CNA's.


Summary of Responsibilities

  • Administers skilled nursing care to patients on an intermittent basis in their place of residence.
  • Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs.
  • Initiates the plan of care and necessary revisions.
  • Initiates proper preventive and rehabilitative nursing procedures.
  • Prepares clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.
  • Counsels the patient and family/significant others in meeting nursing and related needs.
  • Processes orders and notifies physicians of patient needs and changes in condition. Completes certification/re-certification orders and discharge summaries.
  • Refers patients to Physical therapists, Speech-Language Pathologists, Occupational therapists, and Medical Social Worker as needed.
  • Supervises and teaches other nursing personnel.
  • Conducts patient care conferences on patients assigned to his/her care.
  • Takes on-call duty nights, weekends, and holidays, as assigned.
  • Completes and sends OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition.
  • Appropriately uses ICD-10 codes.

Requirements

  • Current Registered Nursing license in Washington state.
  • One (1) year of nursing experience is preferred.
  • Home Health experience, preferred.
  • Ability to travel within a specific region of King County, required.
  • Valid driver's license and insured, reliable vehicle, required.
