Job Summary

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our growing home care team. This full-time/part time RN position focuses on completing client assessments and reassessments to ensure high-quality, individualized care for patients in the Northern Virginia communities. The ideal RN is someone who has a passion for nursing, and great customer service skills.

Responsibilities

Conduct initial client assessments and routine reassessments in client home settings

Conduct hospital discharge assessments

Develop and update individualized care plans

Ensure compliance with care standards

Maintain accurate and timely documentation

Qualifications

Currently licensed Registered Nurse (RN) in state of Virginia

Previous clinical experience (home care, case management, or patient assessment) as a Registered Nurse is preferred

Ability to work independently

Excellent interpersonal and client care skills

Valid driver’s license
