About The Career:

  • The Social Worker is responsible for providing appropriate social services to patients, families, and others identified as having social, psychological, and/or environmental needs related to their rehabilitation admission or adjustment to their rehabilitation or disease process. The Social Worker is responsible for the evaluation of patients; development, coordination, and implementation of the treatment plan; maintenance of informative, accurate, and timely documentation; participation in team meetings; training of others as indicated; and participation in continuing education.

All About You:

  • Education: MSW required or Licensed by the State of Colorado as a Clinical Social Worker.
  • 1-2 years of experience in rehabilitation social services preferred.
  • Support group facilitation experience is desired.
  • Familiarity or experience with insurance pre-authorizations and patient assistance programs is desired.
