Home Care Physical Therapist

Ivy Rehab Physical Therapy / Excel Physical Therapy - Outpatient Homecare

Salary: $105,000 - $115,000

Home Care Physical Therapist - Excel Physical Therapy / Ivy Rehab
Provides skilled physical therapy services to patients in their homes, focusing on improving mobility, strength, balance, safety, and independence. Responsibilities include performing evaluations, developing and implementing individualized treatment plans, providing therapeutic interventions, educating patients and caregivers, documenting care, and collaborating with healthcare teams to support functional recovery.

Key duties:

  • Conduct in-home PT evaluations and reassessments
  • Create and progress individualized treatment plans
  • Provide therapeutic exercises, gait training, balance training, and functional mobility training
