Job Description Summary:
Job Description:
The employee works with staff at referring facilities to identify and assess potential patients for rehab services, conducts physical assessments of potential patients to ensure medical necessity for acute rehab, acts as the primary liaison between the patient, referring facility, physicians, payers and Frazier Rehab Institute. The employee prepares, educates and otherwise facilitates transitions for patients from acute care to the rehab setting, ensures any special needs of the patient are properly arranged for prior to the patient's admission, provides necessary insurance information on potential patients to Insurance Verification and/or Financial Counselor and obtains all necessary precepts prior to a patient's admission, gathers clinical and non-clinical patient information to complete a required online Pre-admission Assessment for all incoming patients. The employee will conduct or coordinate informational/marketing programs to referral sources, as needed, to maximize referrals to Frazier.
Additional Job Description:
Education:
  • Kentucky RN license (associate or bachelor's Degree) or other licensed clinical professional for example PT, PTA, OT, SLP, MSW.

Required Experience:
  • Five years minimum of nursing/licensed clinical experience working with a broad base of patient types/diagnosis
  • Previous rehabilitation experience
  • Knowledge of Medicare, Medicaid, and Third-Party Payer systems

Preferred Experience:
  • Experience with patient/family counseling and problem resolution
  • Marketing and sales background
