As an Outpatient Physical Therapist, you will:

• Conduct comprehensive evaluations that include reviewing patient histories and performing specific assessments to determine individualized treatment plans.
• Communicate evaluation findings and treatment objectives clearly to both patients and their families, involving them in goal-setting processes.
• Implement treatment protocols rooted in current best practices, adapting them based on patient progress and evolving health care guidelines.
• Collaborate closely with a multidisciplinary team of healthcare professionals, identifying changes in patient conditions that require adjustments to therapeutic plans.
• Maintain meticulous documentation of each patient's care, including evaluations, progress notes, and discharge summaries, to ensure compliance with regulatory standards and support continuity of care.
• Manage time effectively to meet productivity expectations while maintaining an organized schedule to accommodate patient needs.
• Participate in departmental initiatives to enhance access to care and improve patient experiences, working collaboratively to identify and remove barriers.
• Engage in ongoing education and professional development to enhance personal competencies, as well as serve as a mentor to students during clinical rotations.

You will be actively involved in the rehabilitation of a diverse patient population, including individuals recovering from orthopedic injuries, surgeries, neurological conditions, and sports-related injuries. Your approach will emphasize personalized care, building strong therapeutic relationships while empowering patients to take an active role in their recovery.

The ideal candidate will possess:

• A Bachelor's degree in Physical Therapy, and a Doctorate in Physical Therapy is preferred.
• A state license to practice as a Physical Therapist as required by New York state law.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• A commitment to ongoing professional development and education.

While prior work experience in physical therapy is not required, candidates with a strong clinical internship background or relevant volunteer experience will be viewed favorably. Familiarity with electronic medical record (EMR) systems and previous exposure to outpatient settings may enhance your application.
