POSITION SUMMARY: Registered Nurses are responsible for providing skilled nursing care to participants. This position is responsible for working directly with the Interdisciplinary Team (IDT), participant, and his or her representative to achieve the goals of the care plan. Communicates to the family or care giver and provides appropriate patient education.

RESPONSIBILITIES AND DUTIES:

ESSENTIAL JOB FUNCTIONS:

  • Completes regulatory assessments of Rocky Mountain Health Care Services (RMHCS) programย participants (initial, annual, and semi-annual)
  • Provides foot care services for early detection of existing and potential foot problems, notifies appropriate provider should foot problems arise
  • Foot exam, including inspection for any discoloration, swelling, redness, warmth, cuts, scrapes, corns, calluses, or other changes of the foot, ankle, and leg below the knee. Toenail clipping, trimming and filing. Callus and corn evaluation. Within scope of practice.
  • Diabetic foot care services within scope of practice
  • Schedule initial and follow-up foot care visits
  • Serves as footcare specialist resource to personnel and orient new personnel to foot care
  • Ensure area is swept, all surfaces are wiped down with surface disinfect spray between each participant, and used instruments placed in a bin with disinfectant solution for soaking and scrubbing
  • Maintain and manage spreadsheet for participants needing routine toenail care
  • Maintain orders tracking within Prime Suite to ensure orders are being completed in a timely manner
  • Communicates effectively to the Interdisciplinary (IDT) team, family, and caregivers
  • Attends and participates in IDT meetings as needed
  • Prepares nursing intervention for care plans and presents at IDT team meetings as assigned
  • Identifies and labels specimens appropriately. Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions
  • Serve as a patient advocate by protecting patient rights.
  • Evaluate the need for a referral to other agencies or providers such as Home Health or Podiatrist.
  • Provide planned education related to prevention, health maintenance, specific disease or condition, and documents results in the medical record.
  • Documents assessments, care, and care coordination within appropriate template in the EHR
  • Provide emotional support and guidance to participants.
  • May provide in clinic, home or telehealth care related to foot care.
  • Coordinate care of the participant by interacting withย departments, ALFs, or LTC facilities.
  • Insure confidentiality for patients and staff.
  • Maintain knowledge by attending appropriate education programs, reading current journals and literature and documents for yearly evaluation.
  • Comply with the Specialty Services incident reporting as well as policies and procedures.
  • Adhere to all OSHA and RMHCS programย training as required per policy.
  • Provide excellent customer service and adhere to Code of Conduct per employee handbook.
  • Must stay current and up-to-date foot care procedures.
  • Must follow current infection control guidelines.
  • Perform other job duties as assigned.

  • Graduate of an accredited Nursing Program (LPN or RN), required
  • Maintain current license as a professional in the State of Colorado, required.
  • Certified Foot Care Nurse (CFCN) or a Certified Foot Care Specialist (CFCS) preferred but not required
  • One year of health care experience in hospital, long-term or home health care setting with 1 year of experience with a frail or elderly population is preferred.
  • Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers.
  • Must have a reliable transportation and must maintain current automobile insurance.
  • Must be familiar and work efficiently with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public
  • Knowledge of medical equipment and instruments
