Job description

Position Summary

The Emergency Medical Technician completes in home assessments for patients recently discharged from the hospital or for patients in need of follow up. This is a fly car position with no ambulance transport. The EMT obtains vitals, observes the home environment, identifies immediate concerns, and connects the patient to the Nurse Practitioner through telehealth when needed. The goal of this role is to ensure patient safety and stability after discharge.

Key Responsibilities

- Complete in home assessments after discharge or during follow up visits

- Obtain vitals and evaluate the patient’s general condition

- Confirm medication access and basic needs

- Observe the home environment for safety concerns or barriers to follow up care

- Connect the patient to the NP during the visit when clinical evaluation is required

- Document all findings clearly and accurately

- Identify non clinical needs and hand off to the CHW or Social Worker

- Communicate urgent issues to clinical leadership

- Represent the program professionally in all patient interactions
