Summary

Were hiring aPer Diem Registered Nurse (RN)to provide skilled home health care services throughoutCasa GrandeValley area.Youll work with an interdisciplinary team and deliver care that helps patients recover safely and independently at home.


Responsibilities:

  • Perform in-home nursing care, including IV therapy, wound care, and assessments
  • Monitor patient conditions and update care plans as needed
  • Manage cases and provide follow-up visits
  • Help patients maintain independence and manage their health at home
  • Perform other related duties as assigned that may not be listed here but are essential to supporting patient care and agency operations
