POSITION PURPOSE
This role involves delivering nursing care within a residential environment for individuals diagnosed with serious mental illness and/or substance use disorder. Responsibilities include facilitating the admission process by screening referrals, conducting assessments, and collaborating with the team to identify immediate and ongoing recipient needs. The position encompasses a comprehensive range of nursing services and involves overseeing the health management of a total of 16 individuals.
POSITION REQUIREMENTS/QUALIFICATIONS
For this role the RN should hold a valid license from the state of Minnesota, with at least one year of prior experience in assisting individuals with serious and persistent mental illnesses. Additionally, the incumbent must meet the following criteria: ability to ambulate and navigate stairways, proficiency in CPR and First Aid with the capacity to respond to emergencies, capability to periodically lift and carry bags weighing up to 25 pounds, possession of a valid Class 'D' Driver's License, a reliable vehicle, a satisfactory driving record, and the necessary insurance.

  • Delivering top-tier, specialized, holistic healthcare services within a rehabilitative and therapeutic care framework.
  • Engaging proactively with clients to cultivate positive relationships that bolster their efforts in achieving personal goals.
  • Collaborating effectively as a team member, taking a lead role in comprehensive health promotion initiatives.
  • Conducting health and well-being education sessions in individual, group, and community settings with a focus on preventive and restorative measures.
  • Utilizing experiential learning techniques as interventions to support the recovery process.
  • Serving as a liaison between clients and external healthcare providers to address holistic health needs.
  • Providing information and/or directing clients to community resources for enhanced support and stabilization.
  • Offering guidance on medication management through coaching sessions.
  • Documenting observable behaviors and symptoms as part of comprehensive record-keeping.
