You'll be working primarily from home while staying closely engaged with our multidisciplinary team, which may include medical directors, nurses, and other social workers. Collaboration is facilitated through regular virtual meetings and case consultations.

• Provide individualized psychotherapy to hospice patients facing mental health challenges, including anxiety, depression, and grief, through evidence-based practices.
• Conduct detailed mental health assessments to evaluate each patient's emotional, behavioral, and social functioning, ensuring that care is tailored to their specific needs.
• Develop and implement customized treatment plans leveraging culturally competent methods, aiming to enhance the quality of life for patients and support their families.
• Collaborate with other healthcare professionals—including physicians and nurses—to ensure integrated and holistic care for patients, attending interdisciplinary team meetings to discuss patient care strategies.
• Monitor patient progress and adjust therapeutic interventions as necessary based on ongoing assessment outcomes and feedback from both patients and healthcare staff.
• Educate patients and their families about mental health issues, treatment modalities, and available community resources, empowering them to take an active role in their care.
• Maintain rigorous ethical standards and confidentiality, ensuring compliance with all relevant professional guidelines and regulations.

The primary focus of your work will be with patients receiving hospice care, who often face complicated psychological challenges related to terminal illnesses. You will support patients through their end-of-life journey, addressing their mental health needs and helping their families navigate the emotional complexities associated with grief and loss.

• A Master’s degree in Social Work from an accredited institution is mandatory.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Arizona is required.
• A minimum of two years of direct experience providing psychotherapy and mental health services, preferably in a clinical or hospice setting.
• Exceptional communication and interpersonal skills, essential for building trust and rapport with patients and collaborating effectively within a healthcare team.
• A proven track record in conducting thorough assessments and developing evidence-based treatment plans.

• Experience working in hospice care or palliative care environments will be highly regarded.
