Job description

OPEN POSITION: 
Physician - Geriatric / Adult Primary Care
SCHEDULE: 
- Full-Time (4 or 5 days per week)
- Part-Time (flexible)
- No Evenings
- No Weekends

- Daily Hours Negotiable
FULL-TIME COMPENSATION:
- $180,000 to $225,000+ Starting Base Salary, depending on experience

LOCATION: 
Cleveland, Ohio
COMPANY PROFILE: 
This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home.  They currently boast a team of more than 30 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office.  Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, and transitional care.
POSITION DESCRIPTION: 
The physician will serve as the primary care provider for a dedicated panel of geriatric patients.
Patients are located in long-term care facilities (ALFs, ILFs, SNFs) and private homes.  Physicians are given a reasonable geographic area in which all patients are located (to ensure minimal daily drive time) and there is flexibility in regard to whether or not house call patients are included in the panel.
Providers see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs.  Dedicated schedulers put together daily schedules and routes for the providers to ensure efficiency and appropriate time for every visit.
Specific duties of the role include, but are not limited to:
- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions and responses to treatment
- adjust treatment plans as necessary
- perform minor procedures as required and as settings permit
- educate patients and families on health management, disease prevention, etc.
- provide guidance on lifestyle modifications and self-care practices
- work with other providers, nurses, and healthcare professionals to coordinate care
The practice uses a mobile-based EHR with AI functionality to reduce administrative time.
On-call is phone triage only with very low volume.  Full-time providers rotate the on-call responsibility, with a shift every 12 weeks or less.
REQUIREMENTS:
- Ohio medical license
- DEA
- Board Certified or Eligible, Internal / Family / Geriatric Medicine
