About the Role:
As a Registered Nurse with Assured Care, you will play a vital role in providing high-quality care to seniors, vulnerable adults, and individuals with disabilities and/or chronic illness in our group home setting in Minneapolis, Minnesota. Responsibilities:
- Provide direct patient care, including administering medications, wound care, and other treatments as needed
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and report patient progress, identifying areas for improvement and making necessary adjustments to care plans
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care
- Educate patients and their families on disease management, self-care techniques, and community resources
- Maintain accurate and detailed records of patient care and treatment
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
Requirements:
- Current RN licensure in the state of Minnesota
- Strong clinical skills and experience in gerontology or long-term care
