Company Overview:

Founded in 2018, Vitae is a growing multi-specialty provider group dedicated to delivering high-quality care to residents in skilled nursing and long-term care communities.

Our clinicians provide integrated services across internal medicine, psychiatry, psychotherapy, and podiatry working closely with facility leadership and a collaborative, multidisciplinary team to address complex medical and behavioral health needs.

Today, Vitae supports more than 300 facilities across the Midwest and East Coast, offering providers the opportunity to practice in a team-based environment while making a meaningful impact on an often-underserved population.

At Vitae, you’ll find a strong clinical network, experienced leadership, and the opportunity to do work that directly improves patient outcomes.

Position Summary

Vitae Health is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ overall well-being and adjustment to medical and life changes.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments for residents upon admission and as needed
  • Develop, implement, and monitor individualized treatment plans
  • Provide individual, group, and/or family therapy sessions
  • Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability
  • Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns
  • Collaborate with the Vitae clinical team, the patient’s attending physician, facility nursing staff, social workers, and rehabilitation teams to coordinate care
  • Participate in care plan meetings and interdisciplinary team discussions
  • Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations
  • Provide crisis intervention and behavioral management strategies when needed
  • Educate staff and families on mental health conditions and effective communication strategies
  • Ensure compliance with HIPAA and ethical standards of practice

Requirements

  • Master’s degree in Psychology, Counseling, Social Work, or related field
  • Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist)
  • 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings
  • Strong clinical assessment, diagnostic, and treatment planning skills
  • Knowledge of mental health conditions common in SNF populations (e.g., depression, anxiety, dementia, adjustment disorders)
  • Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling)
  • Ability to work collaboratively within an interdisciplinary team
  • Excellent communication and interpersonal skills with residents, families, and staff
  • Strong organizational, time management, and documentation skills
  • Familiarity with electronic health records (EHR) systems preferred

Benefits

  • Excellent earning potential with competitive base salary plus uncapped bonus opportunities or flexible fee-for-service options
  • Consistent, reliable referral flow so you can focus on patient care, not building a caseload
  • Comprehensive benefits package including Health, Vision, and Dental insurance
  • Generous paid time off: 3 weeks PTO, 6 paid holidays, plus 1 dedicated day for education and professional development
  • Full malpractice coverage including tail insurance, no out-of-pocket costs
  • Retirement planning support with a 401(k) and company match
  • Ongoing professional growth through CEU opportunities and paid licensure renewals
  • Supportive, clinician-focused leadership team that prioritizes your success and well-being
  • Up to a $3,000 referral bonus, get rewarded for bringing great clinicians to the team
