Position Overview

Horizon Flex Therapy is actively seeking a dedicated and skilled Orthopedic Outpatient Physical Therapist to join our team in Palm Coast, Florida. In this full-time role, you will play a crucial part in guiding our patients on their journey to restore movement and achieve their optimal functional capacity. Your expertise will focus on maximizing quality of life through promotion, prevention, diagnosis, treatment, intervention, and rehabilitation, with a commitment to delivering high-quality care tailored to individual patient needs.

Work Environment

This position is situated within a vibrant rehabilitation facility characterized by its supportive and collaborative atmosphere. Our onsite treatment areas are fully equipped with state-of-the-art tools and modalities that empower our therapists to deliver hands-on care effectively. You'll work closely with a multidisciplinary team of healthcare professionals, fostering an environment of mutual support and shared knowledge. The standard schedule is Monday through Friday, from 9 AM to 5 PM, providing a balanced work-life environment.

Key Responsibilities
  • Assess and identify each patientโ€™s goals and needs through detailed evaluations and consultations.
  • Deliver care to 2-3 patients per hour, providing high-quality, evidence-based treatments that enhance mobility and function.
  • Utilize your orthopedic outpatient experience to offer cost-effective treatment alternatives, reducing reliance on medications and surgical interventions.
  • Develop comprehensive care plans that integrate diverse therapeutic techniques, structured to meet each patient's unique requirements.
  • Create personalized fitness and wellness programs focusing on strength, flexibility, balance, and coordination tailored to individual patient capabilities.
  • Motivate and encourage patients throughout their recovery journey, ensuring they achieve their optimal level of function.
  • Regularly monitor and assess patient progress, continuously adapting care plans as necessary based on evaluative data.
  • Work collaboratively with fellow healthcare professionals to ensure a multidimensional approach to patient care.
  • Document all patient care services meticulously to maintain comprehensive records in accordance with regulatory standards.
Patient Population / Client Focus

You will engage primarily with patients recovering from orthopedic surgeries, injuries due to sports activities, or those experiencing chronic conditions affecting their movement. Your role is vital in ensuring that clients receive rehabilitative care that not only addresses their immediate physical challenges but also enhances their overall lifestyle through educational programs focused on health and fitness.

Qualifications
  • Master of Physical Therapy (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited institution.
  • Current Florida Physical Therapist License with no board or disciplinary actions on your record.
  • National Provider Identifier (NPI) number, or the ability to obtain one before employment starts.
  • Strong manual therapy skills and proficiency in exercise rehabilitation techniques.
Preferred Experience
  • Experience working in an orthopedic outpatient setting is highly desirable.
  • Familiarity with electronic medical record (EMR) software, such as WebPT.
  • Ability to work independently while also being a collaborative team player.
Compensation and Benefits

This position offers a competitive annual salary ranging from USD 120,000 to 130,000, dependent on experience, along with a comprehensive benefits package that includes health, dental, and vision insurance.
