Position Overview

Description:
Licensed Practical Nurse (LPN) - Full Time | 40 Hours/WeekShift Details
  • Schedule: 3:00 PM - 11:30 PM, 5x8 shifts per week
  • Every other weekend (EOW) and holiday coverage required
  • All contracts must run through the holiday season - end date flexible
Position Overview
  • Full-time LPN role providing direct and indirect skilled nursing care for assigned residents
  • Works under the supervision of a Registered Nurse, following defined policies and procedures
  • Maintains a collaborative relationship with nursing staff, physicians, and ancillary staff
  • Must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served
  • Must demonstrate knowledge of the principles of growth and development of the life span
  • Must possess the ability to assess data reflective of the patient's status and interpret appropriate information needed to identify each patient's age-specific requirements
  • Provides care to adult and geriatric patient populations
  • Maintains an appropriate level of knowledge and clinical expertise through continuing education and in-service programs
Job Duties
  • Collects data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicates findings to the RN and other members of the Interdisciplinary Team
  • Contributes to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes, involving the resident/family and healthcare team while promoting a healthy, safe environment
  • Recommends revisions to the plan of care after evaluation of resident outcomes and indicates the need for referrals to ensure all aspects of the resident's care have been addressed
  • Collaborates with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice, assuring customer satisfaction
  • Maintains current knowledge and competency in nursing practice
  • Participates in the mentoring and orientation of staff
  • Considers factors related to safety, cost, and impact in planning and delivery of care
  • Under the direction of the RN, provides leadership in the professional practice setting
Occupational Exposure
  • Occupational exposure to bloodborne pathogens is reasonably anticipated
  • Required tasks involve potential contact with blood, body fluids, or tissues
Required Qualifications
  • Active New York State LPN Licensure
  • BLS certification - AHA or RQI only
  • Minimum 1 year of experience as an LPN
  • Minimum 1 year of experience serving in an LPN capacity at a Long-Term Care (LTC) facility
