Job description

Schedule: Four 10-hour shifts per week within Thursday–Monday, always including Saturday and SundayMake a Difference Where Patients Feel Most Comfortable—At Home

Choice Health at Home is seeking an experienced and compassionate Home Health Field Registered Nurse (RN) to join our growing team.

This position works four 10-hour shifts within the Thursday–Monday period, with Saturday and Sunday required. The schedule provides consistent weekend coverage while allowing three days off each week.

As a Home Health RN, you will provide high-quality, patient-centered care in patients’ homes while helping them achieve greater independence, improved outcomes, and an enhanced quality of life.

What You’ll Do
  • Complete initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs
  • Evaluate patient eligibility and establish the appropriate level of home health services
  • Develop, implement, and update individualized plans of care
  • Educate patients and families about disease management, medications, nutrition, safety, and self-care techniques
  • Determine the frequency and scope of services based on patient acuity and caregiver support
  • Perform preventive and rehabilitative nursing interventions
  • Follow infection-prevention practices and standard precautions
  • Supervise and provide clinical guidance to Home Health Aides and LPN/LVNs
  • Document patient progress and clinical outcomes accurately and promptly
  • Communicate changes in patient condition to physicians and interdisciplinary team members
  • Maintain compliance with payer requirements, agency policies, and quality standards
Qualifications
  • Current and unrestricted RN license
  • Valid driver’s license
  • Current CPR certification
  • Valid automobile insurance
  • Minimum of two years of home health RN experience strongly preferred
  • HomeCare HomeBase experience preferred
