Physical Therapist - Home Health
Make a meaningful difference-one patient, one home, one recovery at a time.
Choice Health at Home is seeking a compassionate and motivated Physical Therapist to join our home health team. In this role, you'll provide personalized therapy that helps patients improve mobility, relieve pain, build strength, and regain independence in the comfort of their own homes.
What You'll Do

  • Perform and document comprehensive initial evaluations
  • Identify functional problems and establish appropriate diagnoses before intervention
  • Develop and implement individualized treatment plans
  • Help patients maintain, improve, or restore physical function
  • Provide treatments designed to alleviate pain and prevent further dysfunction
  • Evaluate patient progress and adjust treatment plans to achieve the greatest possible benefit
  • Follow infection-control standards and universal precautions
  • Maintain accurate, timely documentation in accordance with company guidelines
  • Ensure continuity and quality of patient care
  • Communicate effectively with patients, families, team members, physicians, and other healthcare professionals
  • Support Choice Health at Home's philosophy, policies, and standards of care
Qualifications
  • Graduate of an accredited physical therapy program
  • Current, valid Physical Therapist license
  • CPR certification
  • Valid driver's license and current auto insurance
  • At least one year of home health experience preferred
  • Homecare Homebase experience preferred
Physical Requirements
This position requires the ability to:
  • Stand, walk, sit, climb, crawl, squat, and kneel for extended periods
  • Lift, carry, pull, or push up to 100 pounds
