  • Full-Time Field Visit Nurse in the Northwest side of Chicago and nearby Cook County suburbs, increased unit rate pay/salary options, mileage reimbursement and virtual charting assistant (re-claim your life from charting at all hours with a virtual scribe!) We also have opportuntiies in McHenry and DuPage Counties.

Successful candidates must have at least (1) year of advanced nursing experience and 1 year of Home Health Care experience a plus but not a must, with strong organizational skills to coordinate patient care, verbal and interpersonal skills to support patient relations and communicate with other care team members, ability to follow verbal instructions and read/comprehend written instructions with an enhanced aptitude to perform computer charting. Personal attributes essential to successfully perform this role in assisting our senior patient population with their day to day health needs, this candidate would embody an honest, empathetic, reliable, and caring mindset.

The agency’s Registered Nurse (RN) provides skilled nursing care services by planning, organizing, directing, coordinating, and evaluating all aspects of nursing care in accordance with the physician’s plan of care.

REPORTS INTO Nurse Supervisor

DUTIES AND RESPONSIBILITIES:

Ensures the quality and safe delivery of home health services:

  • Completes nursing assessment forms in a timely manner per agency policy and procedures;
  • Supervises Home Health Aides every two weeks;
  • Identifies physical, emotional and developmental needs of the clients;
  • Develops individual nursing care plan reflects current client needs prior to case conference;
  • Provides health teachings to the client and their families;
  • Makes proper and accurate clinical record documentation that reflects delivery and safe home health care services.

Implements current practice:

  • Assesses client condition on every home visit;
  • Communicates client assessments to appropriate individual concerned;
  • Implements nursing interventions to meet client needs and changed conditions;
  • Documents accurate clinical record on every home visit per agency policy and procedures;
  • Accurately transcribes, implements and evaluates the physician’s plan of care in a timely manner;
  • Implements infection control and safety measures per agency policy and procedures;
  • Monitors client for compliance with defined care plans.

Performs assigned treatment/skilled procedures but are not limited to the following:

  • Initiation and/or administration of IV therapy;
  • Insert and irrigate Levine tubes;
  • Suction and cleans tracheal tubes
  • Complies with the home health agency’s policy and procedures manual.
  • Complies with federal, state, and local regulatory agencies.

QUALIFICATIONS:

  • A graduate of an accredited school of nursing.
  • Must be a licensed Registered Nurse and passed the state board.
  • Has experience in home health or public health nursing preferred but not required.
  • Knowledgeable about current nursing trends and legislative issues.
  • Must possess excellent verbal and written communication skills.
  • Excellent administrative, organization, and leadership skills.

PHYSICAL REQUIREMENTS:

  • Visual/hearing ability sufficient to comprehend written/verbal communication.
  • Ability to perform tasks involving physical activity which may include heavy lifting and extensive bending and standing.
  • Ability to deal effectively with stress.

PROFESSIONAL CONDUCT:

  • Document nursing process, patient response to care, patient status, and progress towards goal attainment completely and accurately.
  • Maintain confidentiality of matters related to patient care and the agency business in accordance with HIPAA rules and regulations.
  • Maintains professionalism in implementing nursing care and establish rapport with patients. Treat patients with respect and dignity.
  • Submit nursing visit notes and other reports on time.
  • Participate in patient care conferences and in-service programs.
  • Visit the assigned case punctually and as per schedule. Call and inform the patient in advance before scheduling the visit.
  • Work cooperatively and communicate effectively with all members of the health team.
  • Participate in educational opportunities to increase clinical knowledge and competence in practice.
  • Perform other duties appropriate to education and/or experience, as may be assigned.
