Hold a small caseload of wraparound families. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.ESSENTIAL DUTIES AND RESPONSIBILITIESWorks as a close team with Program Director, Assistant Director and Regional Managers/Supervisors to ensure adherence to high fidelity wraparound principlesProvides initial and ongoing trainings on high fidelity wraparound principles, phase, and skill sets to staffProvides formal and informal coaching in team meetings, supervisions, and one-on-one to program staffUtilizes wraparound fidelity measurements, and measurement results, to offer feedback to program staff, families, and youth on fidelity efforts and for use in action planningElevates performance concerns of staff to managementProvides coverage for ICC’s and family partners (in event of vacations/leave of absence/termination from program) whose teams they have coached previously to allow for continuity of care, and/or carries small caseload of families - Provides collateral, crisis intervention, rehab, and case management support to teamsProvides high fidelity wraparound training to families, stakeholders and community partnersMaintains accurate, complete, and timely case records per requirements of licensing and private payers, Medi-Cal requirements, and facility policiesRequired to work 3-4 evenings per week for in-field coaching or working with own familiesMaintains clear, effective, and open communication between multi-disciplinary clinical staff, and administrative personnel within and between SDCC programsFulfills training requirements including SmartCare, high fidelity wraparound, cultural competency, HIPAA, and all applicable human resource trainingsMaintains current BBS licensure, associate registration, or certified peer support specialist certification as appropriate.Other duties as assigned by manager/directorLANGUAGE SKILLSAbility to read, analyze, and interpret medical records, journals, reports, and legal documents; Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community; Ability to effectively present information to management and public groupsREASONING ABILITYAbility to read, analyze, and interpret medical records, journals, reports, and legal documents; Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community; Ability to effectively present information to management and public groupsPHYSICAL DEMANDSThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
