DESCRIPTION
We are looking for a Registered Nurse to complete in home assessments for our already existing clients and incoming VA clients.

Your responsibilities
  • Conduct in-home client assessments and reassessments
  • Develop and review client care plans
  • Evaluate clients' needs and help determine appropriate services
  • Provide guidance and oversight to caregivers as needed
  • Communicate with clients, families, caregivers, and office staff
  • Maintain accurate and timely documentation
  • Help ensure quality care and compliance with agency standards
  • Serve as a knowledgeable and compassionate resource for our clients and their families
REQUIREMENTS
