Hourly Rate: $19.25

Schedule: 1st Shift, Flexible (30 hours/week)
Reports To: Competitive Employment Coordinator
Department: Transitional Services



About the Role:

Favarh is seeking a proactive and collaborative Career Coach to support our transitional service program for young adults with intellectual and developmental disabilities (I/DD). This position plays a key role in implementing procedures and individual programs that will enhance opportunities for each individual and preparing individuals for competitive employment through a combination of instruction and workplace internships. The Career Coach is responsible for the on-going support of the Competitive Employment Coordinator in the management of daily operations, service delivery, facility, equipment, transportation, program facility cleanliness, health and safety, and overall implementation of policy and procedures. The Career Coach is actively involved in the organization and implementation of project assignments and actively participates with planning special events. The Career Coach reports directly to the Competitive Employment Coordinator.

Key Responsibilities:

  • Serves as a model Favarh employee while performing duties and responsibilities and in the presence of individuals, their families, job sites and the Favarh community
  • Ability to work a flexible work schedule (5 days) to meet the requirements of Individuals needs to include nights, weekends, and flexible hours. Able to create your own schedule after training.
  • Provide and create any accommodation needed for the individuals.
  • Provides proactive coaching and guidance to job site staff, individuals and visitors daily.
  • Demonstrate good interpersonal skills that enhance communication between individuals, staff, and coworkers.
  • Monitors and records individuals progress through Therap ISP Data entries prior to departing site.
  • Always maintain and ensure confidentiality of individuals.
  • Mileage reimbursement paid for any travel from jobsite to jobsite.
  • Fulfill all Direct Support Professional (DSP) responsibilities at all Transitional Program sites as needed or assigned, including Project SEARCH, River Bend Transition Academy, SEI or Work/Day Transition Programming.


Minimum Qualifications:

Education:

  • High School Diploma or GED required

Other Requirements:

  • Valid Connecticut Driver’s License (minimum of 3 years) with a clean driving record


Physical & Work Environment Requirements:

  • Ability to travel between sites and job placements
  • Work involves variable settings; host site environments, and community-based locations
  • Must be able to sit, stand and walk for extended periods at a time
  • Specific vision abilities required by this job include close vision, distance vision, depth perception and ability to adjust focus.
