Summary
The Social Worker is a leadership position within the Community Residence partnering with the Residential Program Manager, Senior Counselors, Recreational Specialist, Case Manager and Nurse. As a collective, the co-leadership team is responsible for the oversight of the health, wellness, and care of youth in the program. Knowledge of childhood trauma and the impacts is paramount. The Social Worker will identify and assess client and family needs and evaluate, coordinate, and provide necessary services and/or treatment. They will also provide in-home supportive counseling services to residents and families, complete required assessments, and assist individuals and families by helping to articulate goals and providing needed information. Overall, the Social Worker contributes to the Glove House high performance culture by exhibiting our values and providing quality results that keep Glove House as the premier provider of child, family, and behavioral health solutions. The Social Worker is directly accountable to the Residential Program Manager.

Primary Job Functions
1. Provide Behavioral Health Overlay Services (BHOS) interventions under the supervision of a licensed clinician while upholding the Glove House Standard of Care.
Performs all work tasks in a safe manner.

Demonstrates understanding of the four dimensions of safety and can identify gaps.
Treats all children, families, staff and ancillary providers with respect and courtesy.
Demonstrates and models sensitivity to the cultural background of children, families, and co-workers.
Assists in ensuring that job-related activities follow Glove House policies and procedures, OMH relevant professional association, ethical standards, accreditation standards, and the law.
Requires daily interaction with staff and youth ensuring services are delivered in accordance with the Glove House operating modality (TCI), youth's treatment plan, calendars and schedules are followed.
Assists in providing staff and resident counseling and support in crisis situations, including post-crisis response.
Model positive behaviors and implement core values.
Facilitate youth psycho-educational groups.
Provide supportive counseling services through individual, family and group counseling.
Provide case management services, coordinate services with other professionals and paraprofessionals and liaise with outside social service agencies and other organizations, where appropriate.
Assist in writing and maintaining treatment plans, scheduling treatment plan meetings.
Assist in writing and maintain Individual Crisis Management Plans
Complete intake assessments within 30 days of placement. These assessments include Bio-psychosocial, ACES, Sex Trafficking, CRAFFT, Trauma Screens, and Developmental Milestones.
Assist in Discharge Planning for youth and families.
May monitor interns and/or volunteers.
May transport clients to/from visitation or arrange for transportation to appointments, community agencies, etc. as needed.
2. Develop and maintain clinical records and program documentation, such as assessments, treatment plans, progress notes and discharge summaries, according to contract and Glove House standards.
Assures all documentation is completed in a timely fashion (myEvolv, etc.)
Complete necessary documentation in Connections and myEvolv following assessments, treatment plans, individual crisis management plans, therapy, and casework contacts in a timely manner.
Will actively participate as a team member and attend staff meetings.
Participate in all Glove House strategic planning goals, including risk management and Quality Assurance plans.
Must maintain timely compliance with all established Glove House standards and provide support documents as necessary (physical/TB, driver license, auto insurance).

Represents Glove House at state and local forums, developing collaborative relations with other stakeholders in the community and promoting the interests of the agency.
Performs other responsibilities as assigned by the Community Residence Program Manager.
Job Qualifications
Education, Licenses and Certifications
All degrees must be from an accredited university.
Master's degree in social work required, with NYS Licensure as a LMSW within 6 months of employment.
Must have a valid driver's license and driving record in accordance with agency standards.
Experience
Minimum experience 2+ years of working with children and families, child welfare, mental hygiene or the criminal justice system.

Schedule Expectations
The incumbent will work a standard scheduled week, which may include weekends, holidays, and evenings. The ability to work flexible hours is required, as is the ability to provide on-call support when scheduled or to meet business needs. Travel will be as needed to conduct Glove House business-related activities. Generally, duties of this position require 80% time at work (20% is eligible as work from home time). Not all duties will occur within the program (i.e.. There are home visits, groups, education that occur outside of the program).
