Job description

The Home Health Physical Therapist is responsible for providing skilled physical therapy services to patients in their homes. The therapist evaluates patient needs, develops individualized treatment plans, and implements interventions designed to restore mobility, improve strength, reduce pain, and prevent further physical limitations. Care is delivered in accordance with physician orders, applicable laws, and agency policies. 
Key Responsibilities:

  • Perform initial and ongoing physical therapy assessments to determine patient needs and establish plan of care.
  • Develop and revise individualized treatment plans in collaboration with the patient, physician, and other care team members.
  • Deliver evidence-based physical therapy interventions including gait training, strengthening, pain management, balance improvement, and fall prevention.
  • Monitor patient progress and adjust treatment plans as necessary.
  • Educate patients and caregivers on exercises, home safety, and use of assistive devices to promote optimal function and independence.
  • Document all care provided in accordance with agency, state, and Medicare/Medicaid guidelines using appropriate electronic medical record (EMR) systems.
  • Communicate effectively with physicians, nurses, and interdisciplinary team members to coordinate patient care.
  • Participate in case conferences and team meetings as required.
  • Supervise physical therapy assistants (PTAs) and other ancillary staff according to scope and regulatory requirements.
  • Maintain professional licensure and comply with continuing education requirements.
  • Adhere to all infection control and safety standards during home visits.
  • Participate in quality improvement and performance management activities.
Qualifications:

  • Education: Graduate of an accredited Physical Therapy program.
  • License: Current and active Physical Therapist license in the practicing state.
  • Experience: Minimum 1 year of experience preferred, ideally in a home health or geriatric setting.
  • Certifications: CPR certification required.
  • Other: Reliable transportation, valid driver’s license, and proof of automobile insurance.
Core Competencies:

  • Strong clinical assessment and therapeutic skills.
  • Excellent communication and interpersonal abilities.
  • Organizational and time management skills.
  • Ability to work independently in a home-based setting.
  • Familiarity with EMR documentation and OASIS (preferred).
