ChristianaCare's Hope Center is seeking a compassionate and autonomous RN to join our Community Impact Health Team.

In this rewarding role, you'll serve as a trusted resource for patients, providers, and interdisciplinary team members, helping individuals navigate their healthcare journey while improving outcomes for some of our community's most vulnerable populations.

What You'll Do

As a valued member of our care team, you will:

  • Provide evidence-based nursing care to pediatric, adolescent, adult, and geriatric patients in both virtual and onsite settings.
  • Conduct Annual Wellness Visits, Advance Care Planning visits, Transitional Care Management calls, and chronic disease management interventions.
  • Perform clinical procedures including point-of-care testing, EKGs, medication administration, routine treatments, and patient monitoring.
  • Triage walk-in patients and respond appropriately to urgent patient concerns and crisis situations.
  • Serve as a liaison between patients, providers, specialists, hospitals, and community agencies to ensure seamless communication and continuity of care.
  • Educate patients on wellness, preventive care, medication adherence, and disease self-management.
  • Collaborate with physicians, LPNs, MAs, and interdisciplinary team members to deliver an exceptional patient experience.
  • Support quality, safety, compliance, and accreditation initiatives, including JCAHO readiness, POCT oversight, and Safe Practice Behavior Monitoring.
  • Assist with onboarding, mentoring, competency assessments, and clinical education of Medical Assistants and Licensed Practical Nurses.
  • Participate in quality improvement efforts, chart audits, special projects, and clinical process enhancements.

Work Schedule

  • Mon-Fri, day shift with some rotation to late day coverage until 8:00PM.
  • 40 hours per week
  • Weekend and holidays per the guidelines of the department

Education and Experience Requirements

  • A graduate of a state approved or National League of Nursing accredited School of Nursing
  • RN license or eligible for licensure to practice in the state Delaware
  • Two years' experience as a RN, physician office experience preferred
  • BSN preferred; ADN considered with agreement of completion of BSN in 3 years
  • BLS certification is required.

Hourly Pay Range: $38.22 - $61.16 This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
